Male Factor Infertility

Intrauterine Insemination (IUI) for Male Factor Infertility

More than half of couples with infertility involve the man as the cause of the problem. Many couples with mild to moderate sperm problems may be successfully treated through intrauterine insemination (IUI). This is often performed during cycles in which the woman is taking fertility drugs to stimulate multiple egg development.

 

Intracytoplasmic Sperm Injection (ICSI)

In cases of severe impairment of sperm count, motility, and morphology (shape) or in the presence of sperm antibodies, IVF is the best treatment option especially in combination with the technique of intracytoplasmic sperm injection (ICSI). This has revolutionized the way male infertility is treated. Even men with no sperm in their semen can father a child if sperm can be surgically extracted from the man's epididymis or testicle for use in IVF with ICSI. Such surgical sperm retrieval techniques as testicular sperm extraction (TESE), percutaneous sperm aspiration (PESA), micro-epididymal sperm aspiration (MESA) or micro-dissection TESE may be utilized depending on the cases at hand.

With ICSI, a special small needle is used to inject one sperm directly into the cytoplasm of the egg under microscopic guidance. Candidates for ICSI include men with

  • Severely compromised sperm parameters, including concentration, motility, morphology, or antisperm antibodies;
  • Blockage or absence of the vas deferens, where sperm is surgically collected by microsurgical epididymal sperm aspiration (MESA) or testicular biopsy for ICSI;
  • Those who have had low or failed fertilization in prior IVF attempts;
  • Unknown infertility after all diagnostic tests are completed.

 

Management of men who underwent a Vasectomy

In some occasions, couples are unable to conceive because of a vasectomy procedure that was previously performed on the husband. In this situation, only seminal fluid without sperm is released from the husband. To achieve pregnancy, either a vasectomy reversal or IVF-ICSI using surgically retrieved sperm may be done, depending on the clinical background.
