Immunologic Evaluation and Treatment

 

Most IVF practitioners tend to attribute "unexplained" and repeated IVF failure to poor embryo quality and endometrial receptivity, suggesting adjusted ovarian stimulation protocols as a possible remedy. In reality though, there are numerous non-embryologic factors that can be responsible for failed IVF. Immune factor as a cause of infertility remains controversial, but CARMI has included the consideration of these factors in the treatment of infertility only in those with clear evidence.

The risk factors that identify women most likely to have an immunologic fertility factor include:

  • Family or personal history of autoimmune disease such as lupus erythematosis, hypothyroidism (Hashimoto's disease), rheumatoid arthritis, etc.
  • Unexplained or recurrent IVF failures
  • Endometriosis
  • Unexplained infertility
  • Recurrent miscarriages


Upon full evaluation, the possibility of immune factors which may contribute to infertility will be discussed by our specialists. A set of tests will be suggested for those at risk and a recommended set of treatment options will be given for those where medical evidence suggests benefit.
